Point-of-Care Partners, LLC (POCP) is a healthcare information technology (HIT) strategy and management consulting firm. The evolution of ePrescribing is a subject area in which we are considered the premier consulting firm. Our two active practices are eMedication Management and eCare Management.  eMedication Management involves medication adherence, medication reconciliation and post-marketing surveillance.  eCare Management includes personal health records (PHRs), health information exchanges (HIEs), payer-based health records, ambulatory electronic medical records (aEMRs), enterprise electronic medical records (eEMRs) and mobile health (mHealth) under the umbrella of chronic care management.  A more complete picture of our practice domains can be viewed in Figure 1.
